Easier editing of grade categories and items

Allowing grades over 100%

Petr has come up with a clever solution for allowing grades over 100% (MDL-12380) by making use of a new aggregation type, such as ‘Mean of grades (unlimited)’, or ‘Sum of grades (unlimited)’.
